When sending anything to the CAO please photocopy and get a cert of posting which is free.
Documents which you may need to send are:

A P21 for HEAR applicants
Social Welfare Form for Hear applicants
Notice of Assessment (For Hear self employed applicants)
Psychologists Report dated after 01/02/11 for DARE applicants
